Care Coordinator

What You'll Do
- Manage daily rotas across our carer team
- Match carers to clients based on skills, availability, and personality fit
- Handle last-minute changes calmly and creatively
- Be a key point of contact for clients and families
- Conduct service reviews and ensure care plans are followed
- Liaise with healthcare professionals, social workers, and case managers
What We Offer
- Salary: £24,000 – £28,000 (depending on experience)

What We're Looking For
- Right to work in the UK
- Enhanced DBS check (we'll arrange this)
- Previous experience in care coordination, scheduling, or a similar role
- Confident with care management software and Microsoft Office
- Strong communication skills — you'll be on the phone a lot
- Calm, organised, and great at problem-solving
