What Does a Care Coordinator Do?
As a Care Coordinator, you will:
- Create and Manage Care Plans – Develop personalised care plans in collaboration with clients, families, and healthcare professionals, ensuring every detail meets the client’s individual preferences and needs.
- Schedule and Oversee Carers – Manage rotas and ensure all care visits are covered by the right carers, matching clients with carers whose skills and personalities best suit their requirements.
- Client and Family Liaison – Act as the first point of contact for families, providing reassurance, updates, and addressing any concerns with professionalism and empathy.
- Quality Assurance – Regularly monitor care delivery, conduct spot checks, and ensure that all care meets the highest standards in line with CQC regulations.
- Support and Develop Our Care Team – Provide guidance, training, and ongoing support to carers, fostering a collaborative and compassionate working environment.
